The Performance Review Form is an assessment that can be used for employee evaluations. Consisting of questions that are both text based and numeric, an employee can be given a report of areas of growth and strength. The assessment should take about 45 minutes to complete by the manager, and can be utilized for annual reviews, documentation of excellence or failure, and a benchmark for future development.
